Wello Occupational Therapy provides in-home Occupational Therapy services for self-managed and plan-managed NDIS participants across Metro Melbourne.


We offer comprehensive assessments, assistive technology recommendations, home modifications, and practical strategies to support safe, independent living — in your home and community.

Wello OT is not a registered NDIS provider. We currently support self-managed and plan-managed participants only.

Functional Capacity Assessments (FCA)

Comprehensive, in-home assessments to understand your strengths, challenges, and daily living needs.

SIL & SDA Assessments and Reports

Evidence-based assessments for:

  • Supported Independent Living (SIL)

  • Specialist Disability Accommodation (SDA)

Reports document functional capacity, support needs, and environmental requirements to support funding applications and plan reviews.

Assistive Technology (AT) Prescription

Assessment, trials, recommendations, and reporting for:

  • Daily living aids

  • Mobility and seating

  • Pressure care equipment

  • Cognitive supports

  • Complex assistive technology

We liaise with suppliers to ensure equipment is safe, appropriate, and aligned with your goals.

Home Modifications & Accessibility Support

In-home assessments to identify risks and recommend practical modifications, including:

  • Grab rails

  • Bathroom and shower modifications

  • Ramps and access improvements

  • Kitchen layout modifications

  • Environmental safety solutions

Independent Living Skills Programs

Goal-focused capacity-building programs supporting:

  • Home management

  • Self-care routines

  • Meal preparation

  • Community participation

  • Executive functioning and planning skills

Pressure Care & Mobility Assessments

Assessment of pressure injury risk, mobility, transfers, and comfort.

Recommendations may include:

  • Cushions and mattresses

  • Seating adjustments

  • Mobility aids

Cognitive Assessments

Assessment of memory, attention, problem-solving, and executive functioning to guide strategies that support safety and independence.

Mobility & Mobility Scooter Assessments

Assessments to determine:

  • Balance and walking safety

  • Transfers

  • Mobility aid requirements

  • Suitability for mobility scooter use

Our aim is to maximise confidence, community access, and falls prevention.

Billing & Eligibility

Self-Managed Participants

You pay the invoice directly and claim through the NDIS portal.

Plan-Managed Participants

Invoices are sent directly to your plan manager — no out-of-pocket cost.

Agency-Managed Participants

Wello OT is not a registered provider and is unable to support agency-managed plans.

Your Occupational Therapist will complete a comprehensive functional assessment, considering:

  • Equipment needs

  • Independence and safety

3. Personalised Recommendations

Clear, practical recommendations tailored to your goals, lifestyle, and funding needs.

4. Reports & Collaboration

Detailed reports prepared for NDIS purposes, with collaboration across your support network.

5. Ongoing Support

Optional therapy sessions, equipment training, and review appointments to support long-term independence.
